Case Management Services (Social Worker Onsite) 

The City of South El Monte has partnered with Human Services Association (HSA) to offer Care Management Services to our senior residents. HSA will deliver professional social work assistance to senior residents of South El Monte at no cost.

Beginning Thursday, March 6, 2025, a social worker will be onsite, every other Thursday from 9 am to 1 pm at the South El Monte Senior Center, 1556 Central Ave., South El Monte.

Available Services:

• Care Management: Individualize service provide by a professional social worker to ensure that various needs are assessed and met.
• Care Plan: An individualized plan will be completed by the social worker. Problems identified in the comprehensive assessments will be addressed in the care plan outlying the desire solution.
• Comprehensive Assessment:  An in-home comprehensive psycho-social and health needs assessment will be completed for all clients referred to the Care Management Program. 
• Service Authorization: Based on the individualized care plan, the social worker will contact appropriate programs. The social worker will act as an advocate and escort in helping the client select the best program alternative to meet their needs.
